Keeping a daily diary of what's happening to the patient in the hospital, what the discussion is with the doctor, what the plan is for that day and the next day is critically important for documentation, for being able to look back in that document to make sure that what the doctor intended to get done actually gets done. So often, there are such good intentions that doctors and nurses just have but hospitals are a busy place and things can fall for the cracks. And so what a patient advocate can do for their family member or their loved one is to be an extra set of eyes that makes absolutely sure that the care that is intended to be delivered is delivered.
