Dr. Travis Stork: And ladies if you wear high heels, you know firsthand the pain of wearing those stilettos. Well Catherine was in so much pain she decided to undergo a new surgery that promises relief. Catherine: I have an amazing collection of shoes. I used to wear heels everyday. The pain in my feet started seven years ago. After wearing heels, my feet feel like the balls of my feet have been bruised. So at a certain point, I just decided I have to get this fixed. Dr. Ali Sadrieh: Hi Catherine! How are you? Welcome! Catherine: Feels good. Dr. Ali Sadrieh: Good to see you! Come on in. Catherine likes to wear a certain kind of shoe, and she has pain, because her feet have a certain structural problem that causes a lot of pain, and tension to the bottom of the foot. The procedure that we are performing for Catherine is going to be done in two stages. My colleague Dr. Rogers will extract the fat from the abdomen using a very simple liposuction like technique, and that fat will then be refined, so the blood and all the components that we don't want are taken out, and just the pure fat is left. We put that in syringe, and then I inject that into the foot into the area that causes pain. Catherine: All I want to do is be able to put on a pair of shoes and not be in pain. Dr. Ali Sadrieh: These patients can walk in and walk out. Dr. Leif Rogers: We have already numbed up this entire area. I am going to suck the small amount of fat out of here, and the fat we will separate in the back table. She has a little bit of swelling, maybe a little bit bruising, but it's very minor. I am right under the skin, just creating a nice little pocket for our fat to go. This step is important, because she want to have this pure fat as possible. What I am going to do is actually take this fat out of the syringe, and put it on to this gauze. It's got a little bit of blood in there. It's not pure fat. But as the liquid portion gets absorbed by the gauze, the fat is staying on top. So that's how we are actually getting the pure fat. Some people will put this in the centrifuge to separate it. I use this technique. Now you can see this is pretty much the pure fat. This is perfectly good fat for a transfer. This is 3 cc's of fat. That's going to be put into this pocket that I made. I find my original canal there, and I start injecting as I come out, and I can just feel it go in and feel the pad start to fill up. Oh! There is some coming out right here. That tells me that that canal is full. I'll inject on this side now. It's really swelling up, and starting to fill up with some nice industrial strength fat pad. I put three in each foot. That's a lot of fat for a small foot like hers. One little stitch, that's basically the completed dressing that she walks out on. Next week, I will take these stitches out, and she is done!
