Grand Rounds 4.45 by Edwin Leap, MD

Kenneth F. Trofatter, Jr., MD, PhD
Many thanks to Edwin Leap, MD for putting the huge effort into compiling this week's Grand Rounds! I submitted a more obscure offering that was written in response to readers' questions related to FSH and Aneuploidy and am grateful he included it as well.

As women approach menopause their FSH (follicle stimulating hormone) levels rise as the varian hormonal production begines to decline. Since their risk for babies with chromosomal abnormalities rises at the same time, it is only logical to ask if one might cause the other. At this point, it appears that this is not the case - they are coincidental, but unrelated events.

I am 28 and my husband 31. I Have had two miscarriages one at 6 wks (
Feb 2007) and other one at 8 wks (April 2007). I had karotype done and the results were as follow double balanced chromosome translocation between 1 & 5, and the other one between 9 & 13. I know this is rare but I am fine physically, mentally. I started trying again in October 2007 and did not pregnant until this past month, LMP Aug 21, 2008. Now, I am pregnant for the 3rd time and I am in week 8 (50 days). On Oct. 8th, I went for sonogram, to my suprise I saw the baby and the heart beat!! I m extremely happy, but, scared as well, very scared. I do not know if my baby will be healthy. What are the chances? I have a feeling that my baby will be perfectly fine! Otherwise, he would not have made it like the other two. Am I right? But, then again fear fills my whole body at times, after all I have 4 bad chromosomes. What are your inputs? Thanks in advance for your time, and kindness.

To anonymous Oct 10: With that combination of chromosomal rearrangements, the fact that the baby looks good now is a VERY good sign that it is either normal or a carrier of the same balanced translocations. Good luck and let us know how things turn out! Dr T
