Grand Rounds 4.36 at Parallel Universes!

Many thanks to Parallel Universes for this week's rendition of Grand Rounds 4.36 and for including a link to my recent post on the Evaluation and Management of Polyhydramnios.

The evaluation of amniotic fluid is an important part of every pregnancy and understanding the causes, complications, and management of the pregnancy with abnormalities of amniotic fluid, and being able to translate that information into understandable terms for the patient, is a necessary part of the daily routine of any obstetrical provider. I hope both patients and providers will find his series on amniotic fluid useful in that process! Thanks to all of you for reading!

    Thank you for your reply. Here are my numbers, I have my doubts with doing the amnio, can you please let me know if my numbers look good?

    Age at term: 40
    NT:1.2mm
    CRL: 50.4mm
    Gest Age: 11.4wks
    Results:
    NT 0.92MoM
    PAPP-A 0.74MoM
    hCG 1.62MoM

    Interpretation: Screen Positive - Increased Risk of Down Syndrome.

    DS Screening Risk=1:150
    DS Age Related Risk=1:53
    DS Risk Cutoff=1:220

    T18 Screening Risk=1:10,000
    T18 Age Related Risk=1:180
    T18 Risk Cutoff=1:100

    Dr. T-
    Just wanted to share some good news with you. After what appeared to be a cyst rupture/leak a few weeks ago, our baby seems to be fine. At 7w2d the heartbeat was 144! The corpus luteum cyst was still intact, so it looks like it might have been another one that was near it, most likely brought on by the clomid. We're not out of the woods yet... need cerclage at 13 weeks and p17 shots, but things are looking up! Thanks for your advice.

    Adriana: So, your Down's risk is only one-third that based on age alone. That is very good even if it is below the lab's "cut-off.". If you have a normal ultrasound at 18-20 weeks, that reduces the risk at least another 60-80%. Many of my patients would simply wait to have that ultrasound before making a decision regarding amniocentesis. Remember, you do not have to do anything YOU do not want to do! Odds are in your favor that the baby is fine. Let us know how things turn out. Dr T
