Hi, there. I am pregnant after one late term miscarriage, one full term daughter after cerclage, and two subsequent early miscarriages. I was taken by ambulance to the ER on Sunday for pain that was so severe I was passing out, nauseated, and my BP was only 80/50. They found that there was fluid around a cyst on my left ovary and sent me home after observing me overnight. I have now had spotting 3 days in a row... wed, thurs, and today (fri). Last blood draw showed my levels were still doubling, and there was a gestational and yolk sac, but no visible baby yet. I am having morning sickness, but am still worried that the pregnancy is going south again. Is it possible for the spotting to be from the cyst bursting? I don't have another ultrasound until Monday, at which point I should be over 6 weeks and be able to see a heartbeat. Thanks.

Hi miraclebaby: The spotting is coming from inside the uterus. It sounds like you had a corpus luteum cust that ruptured and that can be accompanied by release of the fluid within the cyst and some bleeing internally. That's probably what caused the pain and your body's reaction to that caused the low blood pressure. The cyst that ruptured was probably at the site that produced your current pregnancy. The corpus luteum makes the progesterone that supports the pregnancy until the placenta takes over that function (usually by 10-12 weeks). So, when the cyst ruptured, your progesterone levels might have dropped and that might have resulted in the bleeding from the lining of your uterus (the endometrium). This is not uncommon, so, hopefully, the pregnancy will be just fine. Good luck and thanks for writing. Please let us know how things turn out! Dr T

From what my OB and the consulting OB said, the corpus Luteum is still intact and it was the cyst that had fluid around it... After the leak/rupture it was still over 4.5 cm, which seems like a decent size. I don't think they knew for sure if the fluid was blood or not, and didn't warn me that I might spot from it, but that's the first thing that came to mind when the spotting started a couple days after. I've been on Progesterone supplements by mouth since week 4 due to my history. Thanks for letting me know that spotting after that can be normal. Trying to be hopeful!! I'll let you know how things progress. Thanks again for your thoughts.
